Azelaic Acid Guide
Azelaic acid is one of the most versatile actives in evidence-based skincare. It has documented efficacy for rosacea, post-inflammatory hyperpigmentation (PIH), melasma, and acne. It is one of the few prescription-strength actives that is also available OTC at effective concentrations.
The Three Mechanisms
Tyrosinase inhibition
Azelaic acid inhibits tyrosinase, the rate-limiting enzyme in melanin synthesis. This reduces melanin production in active hyperpigmentation. Anti-inflammatory and antimicrobial
Azelaic acid reduces the population of Cutibacterium acnes in follicles (contributing to acne clearance) and decreases the inflammatory cascade that drives rosacea flushing. The anti-inflammatory mechanism is why it is one of the few actives recommended for rosacea-type skin that other actives (AHAs, strong retinoids) often inflame.
Follicular keratinization
Like retinoids and salicylic acid, azelaic acid normalizes the keratinization process in hair follicles, reducing the formation of new comedones. This makes it useful for acne-prone skin both as a treatment and prevention tool.
OTC vs Prescription: Which to Use
